1) I've heard talk about how @CalNurses and other #CalCare proponents are "unreasonable." That we "won't compromise." That "you can't win that way."

My question to these people is...have you been paying attention at all over the last decade and a half?
2) Rewind to debate over the #ACA. Remember how the initial push was for a #PublicOption? Specifically, a robust one that tied payment rates to Medicare. Not a permanent fix to our health care system by any stretch, but certainly better than we ultimately got.

#ACA coverage has likely broken 41 million, and more notes from the @HHS_ASPE report:

For the most part the ASPE report on #ACA coverage is pretty much in line with my own estimates from January: A bit over 40 million Americans now have healthcare coverage via the ACA via exchange QHPs, basic health plans in MN/NY and Medicaid expansion:

However, the ASPE report has a couple of curious estimates for effectuated QHPs. Overall, they put it at 95.2% of total OEP selections, but this ranges from as low as *78.5%* in New Mexico (?) to *101.3%* in Rhode Island (!)

The first seems way too low; the second is impossible.

However, as @POTUS Biden noted, the upgraded #ACA subsidies of the Am. Rescue Plan & Inflation Reduction Act are currently set to sunset at the end of 2025. It's time to make them permanent. #SOTU
There's also ~2.2 million Americans caught in the #MedicaidGap because they live in states which are *still* refusing to expand Medicaid via the #ACA a decade later.

#FunFact: There's 3 counties in the U.S. where a single 40-yr old earning $35,000/year qualifies for a FREE (as in, $0 premium) Gold* #ACA policy:

Santa Fe County, NM*
Fulton County, PA
Juniata County, PA
*(in NM it may be called a Turquoise plan)

There's 226 counties where a single 40-yr old earning $30K/yr qualifies for a free Gold plan.

There's 619 counties where they can get a free Gold plan if they earn $25K/yr.
Of course, if they earn less than $27,180/yr (200% FPL) they're also eligible for a dirt-cheap #SecretPlatinum plan as well, which they'd usually be better off with anyway.

And if they earn less than $20,385/yr, they're eligible for a FREE #SecretPlatinum plan.

CMS is proposing to shave another 0.25% off the federal exchange user fee. I have mixed feelings about this; it would save enrollees around $18/year apiece on average, but as @BenDAvanzo notes, this is one of the few federal programs that has a dedicated funding source, so...
There's several big wonky sections related to changes to how ACA risk adjustment works. I don't know enough about risk adjustment to comment, so I've just posted the descriptions as is.

CMS is expanding the timeframe for people who are kicked off of Medicaid to select an #ACA exchange plan via Special Enrollment from 60 days to 150 days (60 before / 90 after).

That'll be a huge thing once the Public Health Emergency ends & millions lose Medicaid eligibility.
CMS is also proposing to let exchanges offer an earlier effective start date to avoid people having up to a 16-day coverage gap if they lose coverage mid-month. This, again, is probably mostly triggered by the impending end of the PHE, but it applies in other cases as well.

The re-enrollment change is HUGE. Until now, if you don't take any action, CMS will automatically re-enroll you in the exact same policy, but if it's not available they'll re-enroll you in whatever the closest equivalent to that plan is. For the most part this makes sense... #xp
...but a LOT of people enroll in Bronze plans when they're eligible for a high-CSR plan, often w/the same carrier & the same provider network. Going forward, CMS is proposing to auto-renew people in that situation into #SecretPlatinum plans instead, saving them hundreds! #xp

I picture this scene whenever I think about the glut of "different" insurance policies available in some states via the #ACA exchanges. While too little choice is a real problem, so is *too many choices* when the enrollee can't make heads or tails of the differences between them.

The Washington Post is reporting that "Obamacare premiums are rising 4%" based on @KFF's estimate that *benchmark Silver premiums* are increasing by around 4% in 2023 vs. 2022. 1/ washingtonpost.com/politics/2022/…
That's an important distinction, as *overall* #ACA premiums are increasing by a bit more than that (around 6.2% across 32 states + DC): 2/ acasignups.net/22/10/26/updat…
However, the reality is that for the vast majority of #ACA enrollees, premiums won't be going up by either 4% or 6.2% thanks to the premium tax credits (aka financial subsidies) provided for by the #ACA which are dramatically enhanced by the #InflationReductionAct. 3/
