Yesterday we published our first combined asthma and COPD 'Drawing Breath' @NACAPaudit report.

I thought I'd highlight a few key themes.

First, it's important to note that these data reflect care during the pandemic period when front line clinical teams were stretched...
We have four key recommendations, each relevant to #asthma in adults, asthma in children and young people and #COPD, including #PR services.

The first area is that everyone should be able to access early and accurate diagnosis. Here are some key findings; we need to do better:
The second theme is the need for TIMELY care when a person has been admitted to hospital, for example receipt of systemic steroids in asthma, or #NIV in COPD for those who need it.

Creating awareness for the public on the key factors which influence the lung development till adulthood and sharing the ways to keep the lungs healthy is important.

But now the data suggets that more than half of COPD burden, worldwide is due to causes other than tobacco. Why? What has changed? (3/n)

👀it’s out! The llatest 2023 @GOLD_COPD guidance has been released...

🌟There are BIG changes this year. Here are my first thoughts… but lots to digest:

TL/DR:
1. Revised #COPD defn
2. Etiotypes
3. ABCD is out, ABE is in
4. Farewell ICS-LABA
5. New exacerbation defn

1/n👇
1. Definition of COPD now *includes* emphysema…

'heterogeneous lung condition characterized by chronic respiratory symptoms due to abnormalities of the airways (bronchitis, bronchiolitis) and/or alveoli (emphysema) that cause persistent, often progressive, airflow obstruction’
Etiotypes??👀👇
An alphabet soup from COPD-A (asthma), through 'C (cigarette smoking), 'D (abnormal lung development) and 'G (genetic #alpha-1), to 'I (infections), 'P (pollution) and U (unknown).

What do you make of this? A similar approach has helped in pulmonary hypertension!

"National Lung Cancer Screening Day- USA: Nov12"
(1/21)
The International Agency for Research on Cancer (IARC) is taking the opportunity this November, Lung Cancer Awareness Month, to highlight the burden and principal risk factors of lung cancer around the world.
1.8 million (18 lakh) people died in 2020 due to this cancer (2/21)
This count is almost double the number of deaths caused by the 2nd most common cause of cancer deaths - colorectal cancer!!

Tobacco smoking is the most common cause of lung cancer

Smoking in public places exposes those who are not interested in smoking .. (3/21)

(1/9) #COPD is extremely common among people who use heroin. We studied treatment and outcomes in this important patient group. Diagnosis is often late, and the risk of exacerbations is double that of other patients. New research in @BMJMedicine bmjmedicine.bmj.com/content/1/1/e0…
(2/9) We studied a large cohort of >100k people with a history of using heroin and other illicit opioids in England. In this cohort, the rate of death with a primary cause of COPD was 15x greater than for people of the same age in the general population.
(3/9) Approximately 4,000 people in this cohort were diagnosed with COPD by their GP. We matched each with 5 other patients of the same age and sex at the same GP surgery who were also diagnosed with COPD, but did not use illicit opioids.

Today, @AsthmaIreland made a public statement in support of Minister @EamonRyan's proposed Solid Fuel Regulations. We called on all parties to support the regulations in a bid to end the 1,300 premature deaths caused annually by fine particle emissions from burning solid fuels.
The new regulations would ban smoky coal and wet wood, and prohibiting the commercial sale of sod turf, will save lives, but provide for continued turf extraction by those with turbary rights and distribution within small communities. @Dept_ECC

The Wife is at the hospital for emergency tests (not COVID). I am trying to work, which feels impossible.
I would very much appreciate prayers from those who pray and healing energy from those who don't. (I'm religious, she's an atheist.) Thank you.
We got another test result update today from our fab primary practice. The Wife has no lung disease, which was a serious concern,as she is a recovered smoker & has had a severe lingering cough for months since she had a fall at school in late Nov.(that caused the rib fractures).
We are still waiting on another round of test results due next week on other symptomology, but knowing that The Wife does not have COPD or lung cancer is huge. Thank you again for all your support and well wishes. We are both so appreciative.

🧵on basic #inhaler technique b/c we can all do better educating our #copd #asthma patients 🫁

Studies show inhaler error is common & many pts aren’t taught how to use their inhalers properly

↑💊 ≠ always the answer

#MedTwitter #MedStudentTwitter #Tweetorial
1/
Step 1: Actually ask how they use their inhaler😅

With so many types, it’s not surprising pts & providers get confused 😵‍💫

Esp in clinic, ⏰ is tight. But only takes a min to explain

Also pharmacists/RTs are amazing! I <3 using them when able

2/
Let’s first discuss the common inhaler devices. Which one was used in the clip above?

If nicotine vapes ("e-cigarettes") are, in fact, safer than smoking, and help smokers quit, they are (obviously) profoundly different from recreational drugs like alcohol, caffeine & cannabis.

So, what if we just ask ex-smokers who vape how they're doing?

THREAD
👇👇👇
(2)
Among ex-smokers who quit by switching to nicotine vapes, 93% say their health improved. 62% say "improved dramatically." The 5% who said "no change" said "I was healthy to begin with."

Healthier. This is so different from daily screaming headlines.
(3)
Among ex-smokers who quit by switching to nicotine vapes ("e-cigarettes"), 90% say they now have fewer and less sever lung infections. We're healthier. Does no one care?

Confirmed by these peer-reviewed studies.

Absolutely. Buzzing.

Zoom call with @SFoodbanks, @IanByrneMP, and @Kartik__Raj #righttofood (wow)

These guys are REAL heroes and they dont even know it - their hard work and passion keeps families afloat, and dignity intact, and here's a thread about the importance of this:
2 Adversity hits the poorest hardest - and that includes natural disasters, accidents like Titanic, and austerity: cuts per head are highest for places with the sickest people, especially in the North. Infants of families who are in poverty are more likely to die.
3 We know from @CPAGUK and others that rates of child poverty are unacceptably high - around 1 in 3 kids pre covid, and who knows what now... and that poverty impacts the things we take for granted - birthday presents, school uniform, and even food

My system has been firing for hours since I first saw this thread show up in my feed. I feel absolutely furious that people are misrepresenting #ACEsScience as determinant & dismissing it as unethical & faulty. No one with any reputability is saying "I am my ACEs." #WhyITalkACEs
Maybe you haven't been affected by your experience of #ACEs Maybe you had enough mitigating experiences of #BCEs or #PCEs Maybe you pulled yourself up by your bootstraps. Maybe you were able to access resources in early adulthood to prevent or redirect long term effects.
Maybe the effects of your #ACEs and #ChildhoodToxicStress just haven't shown up in your physiology yet. #Cancer #HeartDisease #Diabetes #COPD #Obesity #AutoimmuneDisease #Suicide #Depression have all been shown to also occur in people with histories of #ChildAbuse #ChildNeglect
