So the problem of MD shortages is so very real. ❤️ the timely article, agree with so much of it, but only partial solutions discussed here. A thread for some more color to this 🧵 1/8

As the MD numbers don’t add up from supply-demand curves, we started a “fellowship/residency” for NP/PAs in critical care & then pulmonary @RKleinpell is a pioneer in this space.

At some point #Telemedicine has to be considered to leverage technology & scale expertise. @ADVISSOR_One is trying to lead this on a national scale

"A variety of clinical manifestations have been associated with EBV reactivation. These include fatigue, psychoneurosis/brain fog, sleep disturbance, arthralgia, pharyngitis, myalgia, headaches, fever, gastrointestinal complaints, and various skin rashes." #EBV
"We found that 66.7% (20/30) of long-term long COVID subjects versus 10% (2/20) of long-term control subjects were positive for EBV reactivation based on positive titers for EBV EA-D IgG or EBV VCA IgM." #EBV / #LongCovid

Why a #NationalHealthID is neither needed nor desirable. Thread...
#AyushmanBharatDigitalMission
1. Indians are already drowning in ID's. Aadhar is already a unique ID.Why do we need another one just for health? Why not use Aadhar? #DigitalHealthMission @mansukhmandviya
2. It is well known that vast majority of people seek medical help within their State or neighbouring State.Someone who is from a village in say, UP,is most unlikely to seek medical assistance outside the State or beyond nearest metro city. Why is a National ID required for this?
3.Less than 5% of Indian healthcare system has Electronic Medical records(EMR). Less than 1% may be actually using it as a point of care system. Digitization of medical records in most hospitals, even corporate ones is limited to typing discharge summaries & storing lab reports

When the #ACA was implemented: there was no mandate established to ensure that #emr #ehr electronic medical records could communicate and physicians could obtain medical records from #doctors caring for the same patients at other offices or hospitals. 1/
What ensued was that the government forces practices to an incur most of the cost of buying expensive electronic medical records and forced them to use them or incur more financial penalties. In retrospect those who did not use #emr and #ehr benefitted 2/
It became impossible to afford an #emr #ehr in a private practice
costs include:
1. Computers
2. Network maintenance
3. Wi-Fi
4. Back-up systems
5. #emr #ehr Licenses
6. Cost of repairs
7. Reduction of time with patients
8. Increase typing-dictating time
9. Longer notes 3/

1/ COVID (@UCSanDiego) Chronicles - May 3, 2020

Lots of news commentary today but first, data: 28 inpatients @UCSDHealth for the second morning in a row (8 on vents); the relatively steady number belies busy admit/discharge activity
2/ @UCSDHealth hit a milestone yesterday with over 10k tests performed. Yesterday just 4 of 540 #SARSCoV2 PCR results were positive, and serology results still ~2%. @SanDiegoCounty added 131 new cases for a total of 3,842.
3/ On the news front...a solid vaccine candidates comes from @ArcturusRx here in @SanDiegoCounty but there's a catch - development was funded by @govsingapore and first doses will go to Singapore. nbcsandiego.com/news/local/pot…

A look at the U.S. attempt to switch medical charts into electronic records. “The system is an unholy mess.” via @FredSchulte and @erikafry KHN/⁦@FortuneMagazinekhn.org/OTI3ODU5
Electronic health records have created a host of risks to patient safety. Alarming reports of deaths, serious injuries and near misses — thousands of them — tied to software glitches, or other system flaws have piled up for years in government and private repositories. #EHR #EMR
It started with the "HITECH" Act and a goal of improving patient safety and creating a system of easily accessible patient health info. The software code was so buggy, that when one glitch got fixed, another would develop, the government found. #EMR #EHR
