Surgeons are more likely to experience infertility, pregnancy loss, non-elective caesarean delivery, and postpartum depression than non-medics. But why is this, and what can we do? 🧵

One aspect is that the average age at planned conception for women doctors is 30 years, 7 years later than the general population. The most commonly cited reasons for delaying pregnancy are career, relationship, and financial situation.

A third of women doctors experiencing infertility said they would, in retrospect, have tried earlier in their careers. At the same time, women who were pregnant during medical school said they felt less supported than those who had completed medical training.

3/ Looking at the study itself, it's a retrospective cohort study that attempted to correct for many factors (confounders) incl: age, sex, geographic location, socioeconomic status, rurality, general comorbidity, surgeon sex, years in practice, specialty, and surgical volume.

And it's not old news. The @RCSnews report led by @HelenaKennedyQC highlighted 'jokes' about rape and sexual assault as a problem in surgery. It noted only 1/4 of respondents reported the harassment/abuse they experienced.

First of all, there aren't enough #womeninsurgery. 13.2% of NHS surgeons are women (ENT surgery = 16.5%). And 35% of surgical trainees are women, compared to 57% of all trainee doctors. (2/15)

If we want to talk about increasing diversity and inclusion in surgery, (a triggering 🧵) we have to normalize female surgical residents having children and lose the antiquated patriarchal idea that we as women are privileged to be joining the “boys club” of surgery so we must
Deny, hide or be ashamed of our biology. There have been numerous research studies and articles on how the rigidity of surgical training disproportionally affects female surgical trainees despite the number of female medical students now surpassing that of males.
This is, in part, is due to the disproportionate burden that women bear of pregnancy and care giving responsibilities. This is something near and dear to my heart-I had three miscarriages during residency-each time on 72 hour call or while operating.

Every time I need to put the cover on the sterile light-handles, I scan the room: “Hi, tall friend! Can you help me?”.

For those of us who work in the operating room (OR), it is immediately evident that the room was designed with a much taller person in mind… a man.
During five years of residency, I thought it was me, that I was too short or my hands were too small. I stood on step stools, and still experienced significant height mismatching.

Oftentimes, I would have hand pain and neck pain for days after a long surgery.

Received an invitation to watch the industry-sponsored lectures for a gynecologic surgery conference. Once again, all men.

Medical device companies & pharma - I am not interested in learning about your products if you can't adequately represent women surgeons in WOMEN'S HEALTH.
Failure of industry to recognize women surgeons as experts contributes to the #genderpaygap as we are not compensated for our time and feedback, and products are made for women without women's input. The top 20 companies that make devices for women have 97% white male leadership
Leaders - please surround yourselves with women & POC so you can present a better product and broader views to a diverse field of surgeons. We are bored with the one-size-fits-all approach.

I recently went through a major change, new job and relocating to a new state with little ones in tow. Like many times before, I was called crazy for making these decisions.

I wanted to share a few things about this experience...
The idea that we’re at the mercy of large entities and that we, as individual physicians, are incapable of bringing about change is false. With the right people, leadership, and with the right goals, we are more than capable of advocating for our patients and improving care.
While we stress the importance of continuous system improvement, what we often neglect to talk about is futility as it relates to changing cultures. When you give your all and your efforts are not only not reciprocated but negated, you might have arrived at the point of futility.

After a week on #COVID19 service, I have some non-clinical reflections for #hospitalists. This is NOT your regular ward time. I’ll be honest, it was a tough week. I’ll start with the tough stuff. But stick with me – ending with more positivity and some helpful hints.
The need for constant vigilance was exhausting. I felt pretty safe in patient rooms with gown, mask, gloves. But on unit, constantly reminding myself to open doors with a paper towel, wipe computer/phone with bleach before using, not touch face: v. tiring. theguardian.com/society/2020/m…
The frustration of knowing certain patients were going to deteriorate and not being able to do anything about it except watch it happen was very tough emotionally. I’ve been spoiled by modern medicine – in general, I’m not used to feeling quite so helpless.

5. Yes, I am wearing my name badge that has MD after my name.

6. Yes, my name badge even comes with a separate badge that lives below my name badge that has MD in large letters.

I'm a chief surgery resident, meaning I lead teams of interns, residents, and students. But even after years at my institution, people sometimes have a hard time seeing me as a doctor. 2/
While my white male colleagues can wear scrubs and patients and staff will call them doctor even if they're a student, I always wear my white coat. I even personally purchased one with thick fabric and fancy buttons with my institution's logo to look as legit as possible. 3/

I always knew that I wanted to become a doctor when I was a little kid. I would play with the doctor’s kit (including stethoscope and scalpel) when I was a little girl and pretended that my Barbie doll was my patient. Little did I know that I’m now living my dream!
Training to become a Consultant Surgeon is indeed very challenging, physically, mentally and emotionally. The long hours, the on call shifts, weekends and nights, clinics, monitoring your juniors, research and postgraduate exams and degrees, PhD.
The never ending work-based assessments, exams, and so forth. There is no end to gaining knowledge in medicine and surgery, every day is a new learning experience. It challenges your patience and virtue, your ability to work under pressure and stressful environment.
