Do anti-seizure medications (ASM) prevent late post-traumatic epilepsy?

1. Traumatic brain injury (TBI) is unfortunately a common cause of disability and death. Seizures after head injury can adversely affect the outcome of patients with TBI. Does early ASM help?

The blood-brain barrier (BBB) is a crucial defense mechanism for the brain, regulating the exchange of molecules between the bloodstream and the brain. When the BBB is disrupted, it can have severe consequences for brain health. (1/12)
Alzheimer's disease is one example of a condition that is exacerbated by BBB disruption. When the BBB is compromised, amyloid beta-peptide deposits can accumulate, leading to #inflammation and disruption of neuronal function. (2/12)

I decided that as well as working at MacBlair, I wanted to be a CrossFit coach. Weekend of 10/11 of Nov ‘18, I was attending a CrossFit Level 1 course in Image
Manchester, to get my coaching qualification (I still passed the course).

It is difficult for me to explain my accident; if you know gym racks 😆 I sustained a concussion and whiplash when I accidently initiated a movement with my legs instead of my shoulders and drove my
head into a three-bar pull up rack (wasn’t a dirty south bar).

The picture 👆 was about five hours after I did it. Not bad eh?!

👇 is why @CrossFit should have stopped me from continuing with the course but that is a whole other part of my story.

2/ The study region has no #REBOA or prehospital resus thoracotomy services. 90.1% of TCA were prehospital with 64.8% from blunt injuries. Only 6.7% had shockable rhythm.

29.9% survived to ICU with mean days on ventilator of 3d. Only 10% survived to 30 days (GOS 3 or 4 mostly).
3/ In-hospital TCA had a much higher chance of survival (42.9% vs 7.0%) - but remember there is no prehospital thoracotomy/aortic compression/REBOA services, therefore I'm not sure how applicable this data is if setup were different. @viseshsankaran @traumakhan @WilsonMSJ

In a recent study on the diet-microbiome-gut-brain nexus in acute and chronic brain injury, findings have shown that the microbiome is affected by neurodegenerative conditions and brain injury states.
While the specific changes to the microbiomes of people suffering from different neurological disorders vary, there is a theme seen in both acute and chronic brain injury. And that theme is that pathogenic and opportunistic bacteria increase and beneficial flora becomes depleted.

Increasing insight in #TBI recovery can negatively impact mood. Mood and emotions are definitely the remit of neuropsychology but all in the MDT can adapt to support confidence, self esteem and mood 1/n

But here’s the thing — they ARE really, really serious.
