My friend @GitaPensaMD just published this brilliant piece in @TIME about the unspoken fear that is driving many docs, nurses, & other #healthcare providers out of clinical care. It’s not what you think. Take a read.

time.com/6267208/doctor…
In tandem, I learned that UCSF nurses are protesting exactly the same conditions that Gita describes: overcrowding, insufficient staffing, a place in which it is IMPOSSIBLE to provide high quality care. sfstandard.com/public-health/…
And last night I had a conversation with a non-medical friend who is experiencing the misery of emergency department overcrowding, first-hand.

➡️Patients & providers, together, must call for better.

One of most important articles I’ve done… showing the noise in clinic BP measurement is large & makes it impossible to track Rx effects; almost useless in evaluating change from 2 clinic visits. Let me explain… ahajournals.org/doi/abs/10.116… @YaleMed @YaleCardiology @CircOutcomes Image
@YaleMed @YaleCardiology @CircOutcomes But first, kudos to the team @yuan_lu1 @shiwani_mahajan @rohan_khera @SpatzErica @GCLinderman #yuntianliu @jbmortazavi #chenxihuang - great team that persisted on this challenging project. And thanks @CircOutcomes @bnallamo for your support and reviews.
@YaleMed @YaleCardiology @CircOutcomes @yuan_lu1 @shiwani_mahajan @rohan_khera @SpatzErica @GCLinderman @jbmortazavi @bnallamo Now, we often see a patient with hypertension & change the meds and have them return to evaluate Rx effect. If everything goes right, drugs and changing doses may affect BP by ~5 mm Hg. Maybe a little less. We wondered how much natural variability there is between visits.

#MondayMorning 🧵

Repeat after me:

Patient burnout is an undocumented public health crisis. 🚨
2) #PatientAdministrativeBurden (PAB), or the work that individuals need to do to get the care they need, is of epic proportions, especially in the context of chronic illness, multiple co-morbidities, life-altering, and life-limiting conditions. 📈

#MissionImpossible #PtExp
3) Patients are buried in:
👉 piles of paperwork
👉 fragmented, manual, poorly coordinated workflows
👉 massive losses of time in waiting and navigating uncharted, unsupported care journeys
👉suffering

Mifepristone approval/access has grown in so many ways:
- Globally access improved
- Indications for use expanded
- Methods of delivery expanding (telemedicine, meds by mail)

In Europe, Germany had the lowest uptake of medication abortion. This might be due to low reimbursement for simple and safe care in Germany and medication abortion is so safe!

I am still trying to understand why #healthcare and #publichealth still use the term ‘hard to reach’. Over the years I have noticed strangers will talk to me-unprovoked- and end up telling me their story or some issue they are navigating. This happened yesterday. 1/ Image
I was sitting on bench soaking up the sun and a man passed by then pivoted back to tell me about the trouble he was having getting an updated ID. This was on his mind acutely because he had just come from the social services office. 2/
He was told he needed a bill to prove his identity but he lives in a building and doesn’t pay utilities. He was headed to a telecom store to figure out how to get a bill for his cell phone- which is pay as you go. In the end he talked to me for at least 15 minutes. 3/

Love this keynote on #digitalhealth #equity @CourtneyRLyles #isrii11. We assume everyone has consistent access to wifi, devices, broadband but many groups who have the most need may not. How can we go beyond our typical spheres of influence to impact health? More info in paper 👇🏼 Image
another takeaway: consider whether relying on a particular research method in #digitalhealth excludes certain groups (eg lower digital literacy, differential access to devices and internet connectivity). consider using multiple ways of gathering data during co-design #isrii11 Image
re #digitalhealth implementation for #healthequity — the setting and context are important! design for the setting it will be implemented in up front, with continuous stakeholder input & partnerships with community from the beginning #isrii11 Image

I'm ecstatic that I was finally able to cover patient design in a paper, as it is a topic I present in each and every one of my keynotes.

jmir.org/2022/8/e39178

This is my first paper as a physician and researcher that I co-authored with a patient scholar, @ePatientDave.
The paper reports that patient design, instead of patient centricity, should lead healthcare leaders in designing products, facilities or technologies for patients.

Patient design means we involve patients on the highest level of decision-making.
While patient centricity is a passive process for patients as whether their opinions would be used in the final design of a product is the decision of those finishing the product.

So the problem of MD shortages is so very real. ❤️ the timely article, agree with so much of it, but only partial solutions discussed here. A thread for some more color to this 🧵 1/8

At some point #Telemedicine has to be considered to leverage technology & scale expertise. @ADVISSOR_One is trying to lead this on a national scale

1) Today, we’re diving into Health Catalyst (HCAT) — a data and analytics business that helps top health systems in the US run more efficiently.

Despite being a public co, HCAT is a classic example of a healthcare company that most people have never heard of…

#digitalhealth
2) HCAT was founded in 2008 by co-founders Tom Burton and Steve Barlow who met at @Intermountain and grew frustrated by the lack of data used to drive the health system.

In the simplest terms, they founded HCAT to reimagine and improve healthcare through data.
3) So what does this actually mean? HCAT helps orgs manage and understand data so they can reduce administrative waste, better manage cost, & improve clinical outcomes.

Although a bit of an eye-chart, this helps highlight all of the different parts of an org that HCAT touches

Your data, packaged and sold by @IBM. 'Business analysts said the data dump instantly makes Francisco Partners a significant player in the multibillion-dollar business of buying and selling sensitive information about the care of patients.’ @statnews @caseymross
@IBM @statnews @caseymross How did IBM get all your sensitive health data to sell? They bought other companies that acquired your sensitive health data. And how did they get it? Mostly from 3rd parties who obtained it from health systems, insurers, & others you trust with your data. #privacy #digitalhealth
@IBM @statnews @caseymross And what is being done w/your sensitive data as billion dollar companies buy and sell it? [well, something that makes it valuable to people who want to buy it] And can you reclaim it or get access to it? [no] And is there consequences to orgs that share your sensitive data [no]

MYTH: ONC’s rule to encourage the sharing of health information, which went into effect on April 5, 2021, is also called the “Open Notes” rule

FACT: ONC’s rule is called the “Cures Act Final Rule” 🧵⬇️
The 21st Century Cures Act was an overwhelmingly bipartisan piece of legislation signed into law by President Obama in 2016. Information sharing is central to ONC’s Cures Act Final Rule and, as a result, some refer to the rule as the “information sharing rule”.
Some also (mistakenly) refer to the Cures Act Final Rule as the "Open Notes Rule” but this is inaccurate. Though ONC supports the concept of 'open notes' and the work being done by @myopennotes, the Cures Act Final Rule and ‘open notes’ are not synonymous.
