Happening THIS MORNING!

We'll be live-tweeting the #Barnraising panel discussion feat. @DrCMBoraas @UshmaU @ajeanstevenson Dr. Suzanne Bell & Dr. Siri Suh! @minnpop #abortion #healthequity
The in-person #Barnraising group is grounding in the political history of #abortion in the US & globally, with @minnpop member Dr. Elizabeth Heger Boyle outlining US restrictions on #RoevWade from the 90s to #Dobbs. #healthequity #ReproductiveJustice
Dr. Elizabeth Heger Boyle continues to define PLACE as a social delineation, a distance, and as physical environment.

@WHO advocates for #abortion across the globe, but that international authority has different meaning in US. #Barnraising #healthequity #ReproductiveJustice

#Populism Poses Dangers to Democracy. It Does the Same to Christian Witness. How polarizing narratives corrupt our hearts and redefine our faith. christianitytoday.com/ct/2021/septem…
"The plot of the populist story is simple, unchanging, and melodramatic: Life consists of a perpetual struggle between “common people” and the elites who exploit them. The former are noble, virtuous, and righteous;
the latter are corrupt, arrogant, and self-serving. The key to eradicating social ills and injustices lies in helping the people recognize their true enemies and mobilize to defeat them." ...What populist rhetoric adds to the mix is fear, outrage, and an all-consuming urgency.

Oh, my misguided friends, let me talk to you about that “top notch medical care.”
(Uh, there isn’t any)
The above tweet is not to say that conditions of confinement in U.S. jails are generally acceptable; they are not. We consistently subject pre-trial (and post-conviction) prisoners to inhumane conditions that violate all sorts of domestic and international law.
It's just that most of America has not historically cared about conditions in jails because the imprisoned population across the country is overwhelmingly African American. #structuralracism

I'm @RRHDr, a #healthequity researcher & the Founding Director of @CARHEumn. My work seeks to manifest racial justice & liberation by making the invisible visible. This means that I start from the premise that racism is a fundamental cause of health inequity. #HardemanTakeover
Today I'm taking over @ia4phs to talk about #structuralracism and health from methodological innovation for measuring racism to its manifestations in our society from abortion access to policing. Structural racism as a #populationhealth crisis. #HardemanTakeover
Structural racism is a public health crisis, but it's also a FIXABLE problem!

Earlier this year @CARHEumn & @minnpop hosted an event on the measurement of structural racism. A product of that event was this 🔥 reading list. #HardemanTakeover

@TheConstelProj - an important new tool for funding transparency & accountability w/ #structuralracism as a public health issue, now a 'hot topic'.

But, remember it's a tool to hold "SYSTEMS* accountable, not just individuals. 🧵 1/6

Tomorrow's session in our doctoral seminar on public health ethics centers "Disability, Ethics, & Public Health."

We began this convo in the fall history of public health seminar via extensive discussions on eugenics and basic notions in critical disability studies.

1/
Tomorrow we'll extend our work by attempt to apply working knowledge of #DisabilityJustice and #DisabilityEthics to public health practice and policy. Last week's application exercise of the REAP Framework in evaluating the influence of #StructuralRacism in policy serves +

2/
As a critical bridge to this week's unit, since of course ableism and racism intersect past and present in all sorts of nefarious ways in public health contexts.

(The app exercise involves continuous Medicaid coverage under the current PHE)

This is happening in U.S. & U.K. Once I got to the management & C-suite, especially in #NYC, I was unwilling to send #HCW into unsafe spaces to be injured or die.

@drbreenheroes this is the crux of the issue. “#Mentalhealth” is really #moralinjury from #ethics violated.
The frontline is disproportionately POC, women. Those making guidelines or in C-suite are able to protect themselves. Leadership manipulates data/definitions, then shames, gaslights. Non-clinicians hired to override #HCW with “opinion.” Those at frontline who speak up: fired.
Who is among these flags (& whose loved ones)? It is not those pushing out pre-prints or on TV nor those safely behind a screen from a second home. It is those at the frontline, working without support, protection, or even without pay (some hospitals asking docs to volunteer).

Our report on #policebrutality shows that the same violent patterns are deployed in different European countries to allow police forces to continue brutalising racialised communities; #structuralracism can no longer be ignored in Europe.
The #Covid19 measures have also allowed police and law enforcement to further brutalise racialised communities. We all remember the #MichelZecler case in France:⤵️
The many systemic human rights violations perpetrated against refugees and migrants in Europe with complete disregard towards the protection of their fundamental rights also clearly demonstrates how institutions in Europe target racialized communities.

Word “racism” silenced, actively, by #MedEd #MedTwitter #healthcare#professionalism” & “communication” labels

Saying “racism” offends or “hurts feelings” > actual #BLM deaths

If #professionalism itself is racist -makes sense that word suppressed

What @ayshakhoury experienced pushes out & silences many

If the editors of @JAMA_current resist hearing word “structural racism” then they certainly are not publishing papers using that word

Nobody likes 'BAME', but how to describe all 'BAME' ppl, not just individual communities where the matter is common barriers, disparities etc. People of Colour (a bit American)? Non-white (negative)? Black as a unity term (old fashioned, embraced less by some communities)? 1/
There's totally an issue with 'BAME/BME/BAEM' hiding even worse stats afflicting some groups, eg ppl of African descent (incl Afriphobia) & Bangladeshi/Pakistani (added Islamophobia). But why can't we use unity terms together with disaggregated data for worst impacted groups? 2/
The danger is losing unity terms because some communities are worst hit by the same issue that's also affecting other communities takes away power from the argument to deal with the underlining causes, fracturing an anti-racist coalition & playing into divide-and-rule. 3/

Title: #StructuralRacism for Doctors- What is it?
1st the title is distressing. Why are we still asking what is it? Why aren't we focused on addressing it? Still defining something that is clear, noted in research, & pervasive is problematic & stalls progress.
Pressing play now
"We discussed structural racism. Going into this interview I didn't understand the concept." I'm confused, who are you? There's no introduction of the narrator. What field is he from? What work does he do? Does he interact with patients & not understand this life & death concept?

It is itself an example of SR:

to have an elite white man, with skin in the SR game on the status quo side, conduct an interview of current saliece, conduct an interview obviously not having made any serious effort to prepare, the hubris & self-serving naïveté is stunning that this was posted. 2/
& for this unqualified person to then go on to appraise from the hip the results of a formal fields of study & policy analysis, completely outside his areas of expertise & with no effort to seriously prepare for this subject,

There are 328,200,000 people in the USA.
Black Americans make up about 13% of the population
There are currently about 877K practicing physicians in the USA....
4% of the 877K are Black
ONLY 2% of physicians in the USA are BLACK FEMALES.
We are out there, serving our communities, taking care of the sick, the disinfrachised. Listening. Doing. Acting.
We are tired. We are only 13% of the population but we are dying faster and worse from almost every disease/disorder...
We know it isn't just race that determines these outcomes. Its a long standing system of #bias #structuralracism and social determinants of health that make this an almost impossible battle to win....

In the UK, @thetimes seems to glorify murderers when they fit a certain profile

The paper is yet to apologise for its tweet (now deleted) & the article, which remains up

It is disgusting hypocrisy & double standards

Reminder: to understand how British media outlets report on terrorism especially when it comes to Muslims:

1. Consistently inconsistent e.g. using terms like "terrorist" inconsistently; &
2. treating Muslims differently to others

See @cfmmuk report:

cfmm.org.uk/wp-content/upl…
Narratives are created in the public conscience when high profile stories of murders & terrorism:

1. Get excused when by white non-Muslim far-right males
2. Get promoted when by non-white Muslim males - often with op-eds

This is how the public are misled

Let's go! 1/
1) When engaging in thoughtful critique, whether your own or someone else's, read the entire paper & supplement.
1x) Critiques of Greenwood et al that raise issues such as SES, comorbidities, etc, imply that the commenter has not read the entire paper. Specificity is key.

2/
2) When identifying limitations of the methodology, differentiate between a) those not mentioned by the authors; b) those mentioned by the authors but not addressed as fully as desired.
