1/ It’s been a week.

It’s well into Friday evening & I just got off the phone with a patient who had urgent radiology results. This was after spending > 40 min on the phone w another patient discussing the same this afternoon.

(18 more to come; stick with me.)
2/ I only got 2 items on today’s long “to do” list done — but that’s on me. I was finally able to sit down for a couple hours uninterrupted and dig deep into a few patient cases from the week.

(And I “don’t work” on Fridays.)
3/ As I hung up the phone, I reflected on how #directprimarycare has enabled me to get to the heart of medicine & the doctor-patient relationship.

During that call, my patient’s spouse broke down and — through sobs — simply said: “Thanks for being so kind.”

I’m LJ, a UK Respiratory doctor & medical education enthusiast. I’m here to share my experience of creating respnetlondon.com #RespEd #RespisBest #MedEd #FOAMed #PressEdConf19 /1
UK medical doctors in training must navigate a complex landscape of changing accreditation requirements. Vital information is often dispersed across multiple emails & websites e.g. JRCPTB & PGMDE #RespEd #MedEd #PressEdConf19 /2
I built Respnet as a practical way to centralise information, but also to inspire #trainees & create a sense of belonging. Belonging, connection & community are important for motivation, fulfilment & #wellness, & help prevent #burnout. #RespEd #MedEd #PressEdConf19 /3

I'm switching my time and effort doing stuff to generate exponential knowledge that cross fertilises domains. Design entrepreneurship for #Socialimpact

The idea is to create knowledge skills and making capabilities, with emerging Exponential technologies intersecting smart devices, cloud computing, Artificial Intelligence, nanotechnology, robotics to create products and services that leverage the democratic power of tech. 2/
It's experimentation all the way. Research is okay, but experimentation and prototyping go much farther in understanding problems better and better. That's the design way. Exploring alternatives over picking from existing solutions. We're trying to impact a billion. #PMxD 3/

It's #BellLetsTalk and for so many who struggle with #mentalhealth concerns, this day raises awareness and builds a culture of #compassion. Let's never again hear someone say, “just snap out of it” or “it’s all in your head.” @Bell_LetsTalk
Each of us may have different levels of understanding when it comes to #mentalhealth. Mental health is neutral. It’s not bad or good. It’s not necessarily positive or negative. It just is. #BellLetsTalk
A tooth ache will put you in the dentist’s chair. The flu is a reason to see the doctor. There’s no shame. Seeking help for #mentalhealth support must also be a safe experience. There's simply no other choice when your mental health is cast in shadow. #BellLetsTalk #EndTheStigma

I wake up to the distant soothing sounds of the M1, thumping my alarm clock to release the sleep midichlorians.

Breakfast is some water, maybe with ribena if I’m having a “cheat day”.
Then I put on my rose-tinted spectacles to block out negative light.

I have a feeling other drivers don’t really appreciate me doing this, but little do they know I’m helping them to appreciate life THAT much more

THREAD: The FAANG-ification of 🇺🇸 #cannabis

There's blood on the streets in #cannabis-land, the latest twist in what's been a turvy year. I could choose from a lot of charts here but chose a global lens and zoomed out to paint the complete picture. Talk about vol. #woof 👇
2/ A few things jump out to me on that chart. The first is the bubble and bust; boy I've seen a few of those in my day. Scary as all hell, all sorts of wealth destruction; never fun. But I also notice the uptrend, which remains in tact. That's sorta a big deal given the vol.
3/ Context matters too. #Cannabis is mind-numbingly a schedule I narcotic-- ILLEGAL. So the growth driving that uptrend has occurred *despite* the best efforts of governments / ruling bodies. Not only is that changing but it will be driven by SCIENCE / EFFICACY / WELLNESS...

“ You are worth saving” #thread

This week’s #10ThingstoKnow on Burnout is by Simon McCormick @DrSimonMc an EM Consultant at Rotherham. Simon spends most of his time teaching as he is in his 2nd spell of ‘burnout sabbatical’ but is slowly working his way back to clinical medicine
Simon has incredible blog site, the Broken Toy (brokentoydotblog.wordpress.com) where he writes about his experience with burnout, some thoughts on wellness and what he has been up to in medical education. Here is Simon’s #10ThingstoKnow about Burnout @DrSimonMc

#burnout #thread
1. What is Burnout?

There is no specific agreed definition so this can make it difficult to diagnose. It isn’t about having the occasional bad day, or even bad week, at work but instead usually involves a sustained negative change physically, emotionally, behaviourally.

On the unofficial start of #residency, here's what I've learned about the crisis of #mentalhealth and #suicide for docs in training.

At the end, I'll share solutions #meded #medtwitter 1/
We think about #depression as an individual problem, but this is a systems problem. Med students are healthier than their peers. Training makes them sick. 2/
At any point in time, 1 in 4 med students is depressed and 11% have contemplated suicide. Only 1 in 6 get help, largely because they fear discrimination. 3/

I deleted a tweet about feeling blue. It’s a valid feeling that we all have sometimes. But I didn’t want to show that side of myself, b/c of the stigma.

But then I thought about it & I’m 100% sure others have days like this. It’s just that we all go around pretending we’re ok.
We force wellness &resilience as if it’s not okay to feel UNwell. I’m here to tell you that even successful people who seem to have their shit together struggle with depression. And that’s okay. There shouldn’t be such a stigma to talking about how we really feel.#ItsOkNotToBeOk
If you are feel sad or depressed it doesn’t mean you are broken or weak; It means you’re human.

Medicine vs. The Machine (thread)

For doctors, nurses, and other healers these are distressing times. The epidemic of depression, suicide, anxiety and burnout that consumes our profession is at least less hidden than ever before. I don't know anyone who is spared.
Let me share a mental model that has provided comfort to me. Perhaps it might resonate with you too. You see, I believe that we exist in two worlds: the world of Medicine, and the world of The Machine.
Medicine is why we are here, why we have worked, sweated, and suffered. Medicine is the calling, and it is an ancient good. There are good days and bad days in Medicine. The days when you save a life, comfort the suffering, support a colleague, make a deductive leap...
