An in-depth review of ACL injury.

If you're interested in sports medicine, you won't want to miss this one!

The ACL is the primary restraint to anterior tibial translation and also plays a role in rotary stability

It is composed of two bundles, named based on their tibial insertions:
✯ Anteromedial
✯ Posterolateral

What structure separates the femoral insertions of the two bundles?
AM bundle:
✯ Tightest in Flexion
✯ Resists anterior tibial translation
✯ Tested by Lachman's/Anterior Drawer

PL bundle:
✯ Tightest in Extension
✯ Rotatory Restraint
✯ Tested by Pivot Shift

The bifurcate ridge separates the femoral insertions of the AM and PL bundles.

Triple hop for distance as a decision to #RTS after #ACLR.
Symmetry in distance masks asymmetry in knee function! doi.org/10.1177/036354…

Why triple hop?
During many sports, it is unusual for an athlete to be required to make a single movement such as an isolated jump or hop.
Triple hop provides additional information about the patient’s ability to absorb and release energy during consecutive plyometric loading. Image

According to Wing 2011, #abstraction in natural language is the act of referring to a set of elements via their shared properties, discarding irrelevant distinctions.

It can be a shape they make ("object"), order of traversal ("control structure") or a condition they all meet>>
From a human communication perspective, speakers make ample use of abstractions expressed in NL, in order to convey complex non-trivial instructions efficiently.

Crucially, the use of such abstract structures in NL is done informally, intuitively, and w/o any formal training >>

Final con edu summary for me for the day (more tomorrow, hopefully):

@BartDingenen on RTP after #ACL injury in football (again, soccer for the Americans) players

#sportfisio2020
@BartDingenen End goal must be known at the start of rehab - what are the individual requirements of the sport?
@BartDingenen Underloaded rehab --> unprepared athletes --> overload --> unsatisfactory outcomes

#ACL injuries in professional football (aka Soccer for the Americans in the crowd - anyone else challenged when they hear this lol) with @MarkusWalden

My note thread to follow....

#Sportsfisio2020
@MarkusWalden "ACL injuries in men's professional football: a 15-year prospective study on time trends and return-to-play rates reveals only 65% of players still play at the top level 3 years after ACL rupture" bjsm.bmj.com/content/50/12/…

FREE to download
@MarkusWalden In their database:
- Few career-ending injuries (1.8%)
- Approx 7% retear rate w/in 1 yr post #ACLR

Are these re-injuries failures? Or a deliberate high-risk decision? Interesting question.

But onward ho! First up with my note thread attached: @DrKateWebster on #ACL RTS and reinjury rates in elite female athletes.

Impt to point out: a lot of data is often on male athletes, not as much known about female athletes!

#sportsfisio2020
@DrKateWebster At ALL LEVELS OF PARTICIPATION, females are at greater risk for #ACL injury.

Overall, elite athletes: 83% return to pre-injury level

Elite females: Also 83%!
(incl handball, soccer, alpine skiing/snowboarding, basketball)
@DrKateWebster Time for RTS:
- No sex difference (Walden et al, KSSTA 2017)
- NCAA Division 1 soccer: cleared at 6 mths (Howard 2015)
- WNBA (Namdari 2011): 11.8 mths

Finally getting to Part 3 of @ACLStudyDay - integrating motor learning into #ACL rehab, with @Dusty_Grooms !

First: required pre-reading:
1) jospt.org/doi/10.2519/jo…
2) ncbi.nlm.nih.gov/pmc/articles/P…

Thread of my notes from the lectures to follow .....
@ACLStudyDay @Dusty_Grooms The brain functions as a hierarchy of networks that promote movement. Everything eventually links to the motor cortex.

Environmental context influences how well a patient looks in different environments.
@ACLStudyDay @Dusty_Grooms Neuroplasticity results in:
- incr cortical area assoc w/ the skill (the more you do a skill, the more your brain dedicates to it)
- new neuron support cells to facilitate skill
- improves neural efficiency (as ability incr, neural activity req decr for more complex tasks)

Finally getting to review a few of the @ACLStudyDay home sessions! First up: Managing ACL injuries and athlete RTS with @clare_ardern! Notes to follow in this thread....
@ACLStudyDay @clare_ardern #ACL rehab goals:
1) protect the meniscus
2) QUADS QUADS QUADS (strength)
3) injury free sport participation
@ACLStudyDay @clare_ardern #ACL Mythbusting:
1) Surgery isn't necessarily needed to get the knee back to "normal."

Rehab alone, early ACLR, delayed ACLR - all had same self-reported knee fctn at 2 yrs post-injury.

So @aaron_s_fox jumped the gate on me :) w/ a thread on the #ACL #injury & #AFLW #sport #womanprobs conversation & he did a great job but I'd like my perspective to come in that special 'tone' I may be known for in presentations... My first twitter thread! Expect sarcasm and gifs
1st, I posted my perspective on #injury #women #athlete #development #biomechanics in another tweet, if you missed it, short editorial: bit.ly/3a5Al1x & presentation bit.ly/32hbogF Read those after this thread of fun, I hope the GIFs help you "hear" my tone :)
I’ve seen a bit about #ACL #injury and #menstrualcycles including news stories about organisations ready to study the menstrual cycle and #training - Let me give you some basic information to help you #factcheck before you start #cheering or #praising
