Early use of high titer neutralisatiing Convalescent Plasma in outpatients reduce hospitalisation. 1/n

So what was wrong with previous attempts at Plasma therapy?
1) Reliable assays for Neutralising Antibodies werent widely available earlier
2) We did not know what titer was high enough
3) Most studies were done in serious ill hospitalised patients 2/n
Why do we need convalescent Plasma in the age of Monoclonal antibodies?
Most monoclonal antibodies are ineffective against Omicron and the ones that retain activity are limited in supply and hoarded by @JoeBiden
Other folks at high risk may need an alternative 3/n

What saved my brother when all hope was God was #plasma. He responded to the 1st unit given to him & the 2nd one got him off oxygen support. Many suffering under this strain have recovered with/after #PlasmaTherapy. Is this coz there's a shortage? #COVID19 #CovidIndia @PMOIndia
Making #PlasmaTherapy illegal is liking tying the hands of our docs & telling them to save lives. Are you trying to kill people? First no oxygen supply & now this!

I ask you, is it harming anyone? Are there any sideeffects? The ministry talks like they have it all figured out.
Shortage of a medicine in the market doesn't render it unnecessary as the govt would have us believe. It doesn't have to be a miracle cure. First, rem's not needed (now DRDO will save us all). Then, why Delhi needs more 02 than Mumbai (asked in SC. new, more virulent strain?!)

I'm collating a thread of resources showing that #PlasmaTherapy doesn't really work & can in fact, lead to the emergence of more virulent strains of #COVID19!

These resources are at varying levels of reading difficulty + reading times needed.

Pls RT and/or add more resources👇🏼
First, here's a short and accessible mega-thread by @netshrink that quotes several other scientists who have also publicly spoken about plasma therapy not working!

@GKangInd @MenonBioPhysics @AnantBhan @vishuguttal

Second, here's a comprehensive article by @sandygrains for @ThePrintIndia clarifying the effectiveness of several potential treatments for Covid, including remdesivir, plasma therapy & more!

#Plasmadonation..one of the most used term nowadays, ofcorse this arouses lots of queries in a donor's mind..

Let's see if this helps to clear the air to some extent 👍summerized in 10 points.

Among the 3 categories of COVID patients (severe, moderate ,mild), plasma therapy is suggested for patients who have been moderately infected.Patients with mild condition often recover easily themselves, whereas the chances of recovery in a severely-infected patient are slim.
When one contracts an infection, especially a viral infection, their recovery depends on the production of antibodies to fight the antigen ( which in the current case, is COVID-19). These antibodies are present in the recovered person’s blood for a few weeks aftr recovery.

.@ICMRDELHI study on #plasmatherapy in #COVID19 pts initiated a debate. This thread collates experiences across India on the therapy starting with #Delhi Health Minister,@SatyendarJain. CM @ArvindKejriwal mentioned 1965 patients got befitted already. (1/n)

Mr. Jain clarified that the @ICMRDELHI study doesn't say there are no benefits of plasma therapy, it says CPT may not be effective for a patient on ventilation. He said plasma therapy does not benefit those at stage 3 of the disease. But it helps those in stages 1 and 2. (2/n)
Doctors across Delhi shared their experiences in treating patients with plasma and commented that the therapy is bringing promising results in absence of a vaccine or other mode of treatment. (3/n)

As several case studies from India produced good results in convalescent plasma therapy, we need more evidence to get to the bottom of this therapeutic aspect.

I have nothing against #PlasmaTherapy for Covid. I've looked at the data. It might be helpful. We need a trial.

Now, imagine if the attention paid to this *one potentially marginally helpful therapy*....

...was devoted, as well (not even "instead") to those sick with Covid. /1
What I mean is this. I'm a believer in biomedical interventions. Heck, I'm a board certified internist. Biomedicine is part of what I do.

But only a part.

So much of the help I try to give patients is not available./2
Not because "it's not my expertise" or any such Pinkerian BS. It's because our priorities are off.

Can we pay workers to stay home?

Can we cancel rent?

Can we give food to the hungry?

Can we make Covid care free?

Darn right we can. /3

Experts continue to stress that the therapy holds promise, and there is a growing need for plasma donors across the country to give a fighting chance for #COVID19 patients with severe symptoms.
➡️What is convalescent plasma therapy?⬅️

It is a passive antibody therapy where the immunity of a recovered patient is transferred to a sick person using blood plasma. When attacked by a pathogen, our immune system produces proteins called antibodies to fight the infection.

“Il plasma, in questo momento, è l’unico farmaco specifico contro #Covid19. Inoculando gli anticorpi dei guariti nelle persone malate è come se somministrassimo l'analogo di un vaccino”.
Giuseppe De Donno, primario del reparto di pneumologia dell'ospedale Carlo Poma di Mantova
La terapia con PLASMA IPERIMMUNE si basa sul fatto che il plasma di donatore contiene gli anticorpi contro il #coronavirus.

As the world is debating over medicines for #COVIDー19, monoclonal human neutralising antibodies are showing promising result in both prevention & treatment. Crisp report by @EricTopol. #plasmatheraphy #COVID #COVID19India
Read 9 tweets
#Plasmatherapy has been tried in SARS, H1N1 and EBOLA outbreaks in past as well. The plasma of patients that have recovered is called convalescent ( kan- val-ascent) plasma and is rich in antibodies (proteins that are produced by human body against infectious or foreign agent) Image
These antibodies neutralise the virus and help in recovery of critically ill by reducing the viral load. Convalescent plasma ( CP) has mainly been used as a last resort in critical cases. In current epidemic also was used in China for a small no of cases and been found helpful. Image
Issues with this treatment are:
1. It has not yet been tried on a large scale in COVID19.
2. Patients who recover from coronavirus infection must volunteer to donate blood.
3. Transfusions carry risk of reaction called transfusion reactions, some of which can be life threatening. Image
