Lots of articles about ICUs reaching capacity, but what does this actually mean?

Buckle up for a #tweetorial about ICU capacity and what it means in the context of #COVID19.

First, statewide measures of ICU capacity can be misleading. Critically ill patients generally go to the nearest hospital. Having "10% of ICU beds free statewide" isnā€™t reassuring if most ICUs are completely full and a few ICUs hundreds of miles away are mostly empty.

Instead of statewide numbers, let's focus on ICU capacity within individual hospitals. Unlike airlines that want to fly full, ICUs are not intended to be full, particularly for prolonged periods.

This weekend I caught up w/ 2 non-medical friends (social distance or phone) and they commented that I seemed frustrated. I think they are correct. As a HCW & leader of HCWs, I feel confident saying we have been happy to be on frontline. This is why we chose these fields.
BUT

I understand the incredible hardship the business closures, stay-at-home orders and other rules have meant for millions of Americans. The economic hardship is terrible, and we must address it as a society.

BUT It is maddening to watch politicians, some in media, & others (e.g. business "leaders") politicize what should be a unified national response based on science & community.
Many using demagoguery & lies to encourage people to ignore basic public health guidance, e.g. masks

A thread about re-opening too soon:

When I discharge a patient from the hospital, we both know their illness isnā€™t over. Theyā€™re simply well enough to no longer need 24/7 nursing care or IV medicines. 1/8
When I send a patient home from the hospital, the goal is for them to stay home. Worst case scenario for me or them is to get worse at home and have to come back in. That is MY fault when it happens. 2/8

Thread: Something happened in my home state of Michigan today that made me sad. I wasn't going to comment initially, but then I went to Facebook & saw people I grew up w, who know my husband and I are physicians and call us for medical advice, were defending these actions.

An update on funds raised thru twitter this week. I had a broker in China who had 5k masks; tried to get them to Elmhurst hospital. Customs/FDA stoppage broke deal. Of $9k raised, donated $7k to getusppe.org, $2k directly to Elmhurst. None remain. All receipts below.

Shedding may occur up to 48 hours before symptomatic. Asymptomatic may have lower viral load, and may not be as infectious, BUT STILL DO SHED AND ARE INFECTIOUS.

By 14 days, most people stop shedding.
