Some unpopular opinions for new medicine interns that I wish someone told me one year ago:

#Interns #MedStudentTwitter #Medicine #MedTwitter
1. When you get your schedule, you'll hear that you’re starting on the worst block/service/etc.

Ignore that. Like med school, intern experiences depend on unpredictable factors like who you work with, availability of resources, proximity to free food, etc. It's super variable.
2. People will tell you to separate work and home.

Instead, do whatever you want. If you pre-chart better at home or want to note-prep before going in, if it helps you know what's ahead, do it. Check on your patients on your day off if you want. It's okay to worry about them.

How it started...

On this day, two years ago, I was hit head on by a distracted driver on the country highway by our home. My husband died instantly and I was careflighted to the local trauma center where I remained in critical condition. It was my fourth year of medical school
Over the next few months, as I dealt with the grief of losing my husband, I struggled to overcome my injuries - internal bleeding; open fracture of the femur, tibia, and fibula; and open fracture of the 5th digit of my (dominant) left hand at the MCP and DIP joints. I worked to
independently accomplish my activities of daily living when sitting on the side of the bed, transferring myself to the bedside commode, or feeding myself were feats in itself.

Last night, I worked my first night shift as an OBGYN resident and I find myself crying in realization

What is the diagnosis?
In patients with cirrhosis, always try to find the cause in those who present early. There is no idiopathic, no cryptogenic. Its all 'missed' cirrhosis.

1/ #Empathy tweetorial: I overheard an awesome narrative building, empathic conversation enroute to the hospital today. Two #interns merged behind me (both wearing masks, btw):
Him: “How has your week been so far?”
Her: “It’s been pretty good but sorta overwhelming”
2/It could have ended there with him saying, “That sucks,” or “Me too."
Instead, he said, “Why was it overwhelming? What did you have to do?”
Her: “I went to meetings that reminded me of how many things I have forgotten. (insert details about what she had forgotten)”
3/He could have said, "Yuck, I hate meetings."
Instead he said, “Yeah, that can be scary. What else did you have to do?”
Her: “I took care of some patients with illnesses that I have never taken care of before like COVID. (insert details about patients and illnesses)”

Thinking about a low cost easy to manufacture home test kit for #Coronavirus. Outline: #Nasal_swab (Q-tip) w/ freeze dried reagents for isothermal #DNA amplification with #COVID19 primers. Colorimetric readout by #iOS or #Android #App w/ #geolocation & #HIPPA compliant reporting. Image
Read-out would be in minutes - timer in App - read by camera in your smart phone. No other machines. Links to telemedicine groups for positive results. Built in positive control - e.g. no color = improper test, green NEGATIVE, red POSITIVE.
I’ve done my best creating when faced by a challenge. Next gen sequencing when my son went to new born intensive care - #454 @iontorrent. Ultrasound on a chip @ButterflyNetInc when wanted to monitor my daughter’s kidneys.
