**Fever-prevention and Temperature Control after Cardiac Arrest**

Tomorrow, an important intervention from #BOXtrial will be presented by my supervisor prof @CHassager from @cardiac_group:
Device-based prevention of fever 🌡️ (after 24 hours of #TTM). This requires a tweetorial
Targeted temperature management (therapeutic hypothermia, now temperature control) has been the cornerstone of neuroprotection in patients resuscitated but comatose after #OHCA for the past 20 years
The exact mechanism of action is unknown.
Experimental data suggest that low temperature decreases the cerebral metabolic rate, and consequently reduces the release of excitatory aminoacids and free radicals.

THREAD and my infographic of the #TTM2 trial

#OHCA patients were randomised to #TTM at 33°C or normothermia (<37.8°C) with early fever treatment.

➡️ Mortality and poor neuro outcome at 6 months were not lower in the hypothermia group.

Guidelines recommend selecting and maintaining a constant target #TTM between 32 and 36 °C in comatose post-cardiac arrest patients to prevent hypoxic-ischemic brain damage.
(strong recommendation, moderate-quality evidence). #FOAMcc Image
Two RCTs published in 2002 showed an improved survival and neurologic outcome in patients resuscitated from OHCA of presumed cardiac cause and shockable rhythm who underwent hypothermia at 33°C. #FOAMcc Image

2020 has been extraordinary.

But we wanted to share the 10 reasons we are proud of 2020 by the S-Lab research group of @PICU_BCH & @BacrUob @unibirmingham

AND to say a HUGE 'thank you' to everyone who has made this possible #teamscience #PedsICU #2020
1)In the last 12 months we have built up a team of Neuro-critical care, resuscitation science, early rehabilitation and COVID19/PIMS-TS researchers for in Birmingham to improve the lives of critically unwell children. Currently called the S(cholefield)-Lab pending a better name!
2)In 2020 we watched our early career researchers shine

Ladies and gentlemen, i've got a series beginning today called #KnowYourOwner

I wondered how many people know about the people behind their club. The money, passion, scandal and more. So every Friday, i'll drop a thread.

I begin with @TTM_Original

Thread 👇

As usual, I will re post my disclaimer. I'm not an auditor, accountant nor am I a sports journalist. I am merely a football fan who likes to read up on the financial side of sport and this is all publicly available information.

Let us begin

#KnowYourOwner
#TTM is owned by a consortium called the Black Gold Family Trust. There are said to be four members but the figurehead of the trust is one Masala Lawrence Mulaudzi, a native of Tshakhuma village in Limpopo hence the name, Tshakhuma Tsha Madzivhandila.

Damn, Wits is gone. It's a sad day for football in South Africa with the announcement that #BidvestWits, a 99 year old club, has been sold to #TTM

Here's a thread on the history of the club
Wits University was formed in 1921 by the university's Students Representatives Council. The club competed in a variety of tournaments and leagues before eventually winning promotion to the National Football League in 1975
Wits won their first major title in 1978 – winning the Mainstay Cup after beating Kaizer Chiefs in the final of the competition. Six years later they picked up the BP Top 8, and a year later in 1985 they again beat Chiefs, this time in the final of the JPS Knockout Cup.

Don't be fooled by today's brief flurry to the upside. A lot of work has to be done before Tata can be considered bullish. Keep on selling it.

Chart 1
Monthly Chart: Price hasn't close above the SMA 20 since July 2017 and even a close above doesn't mean much, because there exists a #confluence of Fib and pivot resistance residing higher and broken #trendline support now turned resistance.
