Interesting new paper by @DEOlsonLab, @LinTianPhD, et al. looking at why some serotonin 2A receptor agonists promote neuroplasticity, but others do not.

Various small molecules, from endogenous neurotransmitters like serotonin to tryptamine #psychedelics, activate 5HT2A receptors... and yet they can lead to very different effects.

Getting at why this is was one of the basic motivations for this study.

One idea here is that various compounds have distinct physical/chemical properties, despite all activating 5HT2A receptors.

For example, they differ in fat solubility. Some can cross cell membranes to get *inside* cells, and some can't...

Interested in the effect of #psychedelics on the brain? Did you know you can image the cortex and extract valuable physiological information at the same time in an ecologically valid setup? We used @kernelco Flow1 to do exactly that! A #tweeprint:
Flow1 is a Time Domain (TD) fNIRS device, which measures cortical brain activity (oxy- and deoxy-hemoglobin concentrations) and physiology such as pulse rate (PR) and pulse rate variability (PRV).
In our study, 15 participants underwent subanesthetic #ketamine and saline injections. First, we found that ketamine administration caused significant changes in systemic physiology: increase in PR and decrease in PRV.

2/n We were guided by clinical trials showing that psilocybin improves symptoms of depression and anxiety when given during psychotherapy or other clinician-guided interventions
3/n Our thinking was, what if this is not about a classic drug effect per se, but about psilocybin creating a behavioral state that improves the outcome of therapeutic intervention by relaxing prior beliefs and promoting flexible thinking?

Now that #psychedelic trial's lack of blinding is hot again, lets remind ourselves that for most treatments blinding doesn't even work as a concept, eg. mindfulness, psychotherapy, lifestyle interventions etc. Lack of blinding is the rule rather than the exception, a short 🧵
Blinding as a concept only works for pharmacological treatments, but there are many more interventions where 'blinding' does not even work conceptually (see 👆). Nobody is upset that blinding quality is not measured and blinding integrity is not maintained in an exercise trial.
The tension:
-psychedelics are pharmacological (lets set aside therapy for a sec), so blinding should work
-blinding obviously does not work due to strong subjective drug effects
Its an 'unblindable' pharma treatment (at least by conventional methods), which is strange at first

In the @COMPASSPathway trial the 25mg vs 1mg (=placebo) #Psilocybin difference is not only significant statistically, but also clinically. On the MADRS the 'minimal important difference' is ~3-6 points (tinyurl.com/5apzv8h8), the 25mg dose meets this criteria (more context 👇)
The 'minimal important difference' sounds like a low bar to cross, but actually most #antidepressants fail to do so relative to #placebo, tinyurl.com/yut6xyhd - thank you @PloederlM for your work on this, would love to hear your take on this trial!
Lack of blinding remains an issue, but the dose-response relationship should alleviate this concern. As I argued before, #psychedelics macrodose trials most likely will always lack blinding due to obvious drug effects, it is the nature of the intervention.

The 'default mode network' (#DMN) is the neurobiological poster child for #psychedelic #drugs (e.g., see #HowToChangeYourMind). A preliminary meta-analysis from the @PsychedelicUNIT suggests this might be overstated, w/ other regions being more impacted, like ye olde cerebellum. Image
These results are from 'classic' #psychedelics like #LSD and #psilocybin taken by healthy people during resting-state neuroimaging. We used dose as a covariate. A possible caveat is that we mixed blob and functional connectivity analyses which we will now try and model.

Despite the importance of #blinding in medical research, most trials don't assess blinding integrity, partially because there is no method to adjust trial results for blinding integrity... until now! New preprint with implications for #microdose and #psychedelic research 🧪🧵👇
First, we define activated expectancy bias (AEB), which is an uneven distribution of expectancy effects between treatment arms due to patients recognizing their treatment allocation. AEB can be viewed as residual expectancy bias not eliminated by the trial’s blinding procedure.
The main idea behind AEB is that if treatment allocation can be deduced by participants, then, treatment expectancy can bias the outcomes in the same manner as it biases non-blinded trials, for example as in open-label trials. Image

Although some have speculated that #psychedelics produce enduring enhancements of cognitive flexibility, here we finally show that psilocybin therapy enhances cognitive and neural flexibility in major depressive disorder. However, there's a caveat. 1/10
