9 years of disaster. There are so many instances, that it would be tough to pick just 9.
Here's my list.
1. #Demonetisation. That one stroke of stupidity that literally broke the nation's back. ImageImageImage
#NaakamiKe9Saal
2. #Pulwama
Still remains a mystery, how a vehicle loaded with explosives could move around, undetected, in J&k, one of the most militarised regions in the world. What was more shameful than this was how BJP used this tragedy for electoral gains.
+ ImageImageImage
#NaakamiKe9Saal
3. #JammuAndKashmir
Art. 370 was abrogated. J&K stopped being a state. Was carved out into 2 UTs. With the objectives of bringing peace & development to the region. Result? We've ended with 2 UTs with seething discontent below the surface.

#COVIDSecondWave care requires local #CommunityEngagement for it to be meaningful, respectful and dignified; at one of #chamarajanagar #solega #adivasi settlements yesterday was a humbling experience learning together with the people how to organise care in a culturally safe way
A remote forest settlement; sudden spurt of cases; testing hesitancy and immediate referral to far away #covidcare centres driving anxiety for testing and failing trust in ASHAs; #Panchayat president on location, NGOs, health workers, tribal welfare officers on site
Hours of respectful dialogue follows; various modalities discussed; an amazingly enthusiastic #Panchayat president convenes impromptu consultation at local tribal residential school; CEO and Zilla panchayat admin patiently listen to local proposals

(1/n) Thread: Epi-immuno-pathogenesis of #BlackFungus, misnormer for #Mucormycosis.
As of 26th of May 2021, India had noted close to 12,000 cases of Mucor along with the #COVIDSecondWave.
The centre has advised it a notifiable disease under Epidemic act.

(2/n) Pre #COVID19India, Mucor in India was almost exclusive to patients with uncontrolled diabetes, and usually presented along with DKA.
The other subset was among profoundly immuno-compromised hosts and iron overload states.
(3/n) Interestingly, different etiological risk factors cause various clinical presentations of mucor.
- Diabetes with Rhino-orbital disease
- Solid organ transplant recipients with pulmonary disease
- Disseminated disease in hematological malignancies

**3 must remember warning signs**

- High grade fever even after 5 days
- Increasing dry cough while sleeping, talking
- Exhaustion, exertional dyspnea (worsening from before)
*3 reasons for late presentation*
- Attributing fever to post vaccination and waiting( post vaccination fever is never more than 48 hrs)
- Using DOLO/PCM 6 hrly x 1week as a drug, masking fever
- Wrong timeline, reporting testing day as day 1, not onset of symptom.

Marriage season in #TamilNadu has begun, and if people do not confine it to a very close family event, there are possibilities that weddings emerge as super spreaders. The situation could be alarming in rural areas. (1/n)
Government has put a cap on the number of people for marriage, but that is not followed in many places. In villages, marriages are held at the homes of either the bride or the groom. They are normally closed spaces. And neither social distancing nor masking is followed. (2/n)
Some 100 people will easily gather in a house for the wedding. And in villages, food will be served in houses that are close to the venue. The spread will be rapid of proper measures are not taken. Many people are shifting weddings to their residences from marriage halls (3/n)

My plumber who applied for an e-pass to fix some sanitary works at my parent's home, but Police rejected his application!

Why should promise that plumbers and electricians are allowed when Police outrightly reject the applications?

#Kerala #lockdown2021
I have helped him to register for e-pass and looking It every hour and today morning seen his application rejected.

It is true that more than 2 lakhs people applied for e-pass, but the rejection looks so arbitrary considering the exempted people who are denied the e-passes.
lt is difficult for these small plumbers, electricians, home makers to prove that their request is genuine as they have no documents to prove that they are engaged in the respective profession. They have other IDs but that does not indicate their profession.

For the first time since the Modi Government came to power in 2014, senior BJP and RSS ministers are on the defensive, unsure of how or what to communicate to the people as the #COVIDSecondWave rages across the country.

@ravishkt writes: indianexpress.com/article/india/…
The common refrain is that it was “sudden” and “unexpected,” even when told that the uptick began in Mumbai mid-February and all warning signs — from need of oxygen to the possible impact of the variant — came early. The Government had time to prepare. (2/10)
Asked about the conspicuous silence at the top as compared to daily and regular press briefings by some Chief Ministers, a Union Minister said: “I am with you on the need for a voice of reassurance (from the government). The entire situation is challenging”. (3/10)

A week ago, we had a 20 yr old woman in the ICU, who was 5 months pregnant. She was a high risk patient. We tried saving both the mother & child. Obstetricians, intensivists, anesthetists all were extensively discussing her line of treatment.
(1)
One day, while on rounds, her O2 saturation started dropping. We increased the O2 flow, calmed her, asked her to take deep breaths. She became stable only to deteriorate a few hours later.
With no option left, we had to sedate her & put her on mechanical ventilation.
(2)
There she was, a 20 year old, who was soon going to be a mother, lying on the bed with tubes in her mouth and IV lines running from her hands. We waited for her to get better in the following days, but ultimately had to deliver the baby through a C section.
