I've been thinking about compassion and pediatricians.

There's an idea that pediatricians are "softer" than internal medicine docs because we take care of kids. And kids are cute! Funny! Sticky! 1/
Once when I was a resident, a scrub tech told me I was going to treat runny noses all day while HE worked in the OR doing SURGERY.

I had already matched to a hem/onc fellowship.

But the perception was there. My job was soft. His was hard.

The irony is that pediatricians embody fierce compassion.

We show up when kids have been shot. When they have been abused. When they have been disfigured.

When they are dying.

I am currently at home in isolation with (mild) COVID. Iā€™ve been thinking about why kids & teen COVID vaccines AND BOOSTERS are really important, right now. Here is my long šŸ§µ:
Pediatricians deeply understand risk/benefit of vaccines. As a peds ID doc, I see kids who fall through the "prevention cracks" including immeasurable suffering from severe tetanus, measles pneumonitis & SSPE, chickenpox encephalitis, pertussis, meningitis, and now COVID.
Situations of telling a gasping teen to ā€œlay proneā€ are vaccine-preventable. This pt was also alone, because her parents were at home, also sick w COVID pneumonia, and too ill to accompany her on hospital transfer. They were all previously healthy and unvaccinated.

Weā€™d ditch glib mortality statistics that dress up data as science + paint a more complicated picture.

Weā€™d reassure you that children continue to mostly do well with acute covid. Weā€™d then explain that deaths + hospitalization from acute covid are just part of the equation.

Team it's time to address the Hoeg et al preprint on post-COVID vaccine myocarditis. How am I qualified to speak about myocarditis? I am board certified in paediatric cardiology and currently have patients recovering from this condition. TLDR: They are doing quite well. #MRI
What is the premise of that article? directly scanning VAERS data will result in capturing more cases of myocarditis, and therefore the "conclusion" that COVID vaccine generates a higher risk of myocarditis than COVID disease. But - cardiologists have diagnostic criteria. #heart

To reassure parents, a reminder of some key facts about #COVID in #children:

#SARSCoV2 infection remains asymptomatic or mild in the vast majority of children, even with the #DeltaVariant. Hospitalisation and deaths from COVID are still rare in this age group.

However, the greater transmissibility of the Delta variant means there is more COVID in *all* age groups, including children, so we will see more paediatric cases.
As a large number of older people are now vaccinated, the increase is blunted in the elderly so, compared with earlier in the pandemic, a higher *proportion* of current cases will be in children: this can give the mistaken impression that the Delta variant is targeting children.

We are officially back to getting crushed by #COVID, and for #pediatricians this wave feels *worse* in many ways. And as a pediatric hospitalist who has been a front-line COVID doctor for the entire pandemic, Iā€™m going to share what Iā€™ve seen the last 8 days on service. 1/
After many months of zero or few pediatric COVID cases, we are seeing infants, children, and teens with COVID pouring back into the hospital, more and more each day. These patients ranged in age from 2 weeks old to 17 years.
COVID admissions in our medical center are up 500% and currently 80% of cases in our hospital are due to the highly contagious Delta variant. I worry that the Delta variant, in addition to being more contagious, *may* also be more virulent in children.

Iā€™ve been bombarded with DMs, comments, articles arguing for a back to normal for kids that ditches all masks for kids + insists covid isnā€™t bad for them.

The main tactic is the same old using stats + missing the big picture to ultimately argue covid isnā€™t dangerous for kids
The reality of covid risk to kids is much messier. It involves understanding the complexities of mortality rates, how all children deserve to live a life free of vaccine-preventable disease, how the known + unknown long term effects scare pediatricians more than acute disease

Itā€™s the first week of residency for many incoming residents this week. I remember my first day @usask so vividly: I was so stressed out that I developed tinnitus and asked my senior to examine my ears because I thought I was suddenly becoming deaf. Everyone: take deep breaths.
I have one advice: you are NEVER alone. Donā€™t ever think that you need to suffer in silence. If you donā€™t know something, itā€™s normal! Youā€™re not supposed to know anything: thatā€™s why youā€™re where you are. No one expects PGY1s to be experts in their fields.
Your co-residents will become your family. You will look after each other. That is so precious and will last your entire career. But also remember that your staff is here for you: you need a shoulder to cry on? You need a hug? You need a warm supper? Weā€™re all over this. Call us.
