🧵I’m a little over half way through reading @R_Denhollander ‘s book, What Is A Girl Worth. I’ve had to take it slow. Even though I was married to my abuser and the circumstances of my assaults were very different from hers, the book has proved triggering.
But I keep reading because when she describes her feelings and responses to trauma, she mirrors mine. All those fears about coming out with the truth, trying to navigate motherhood while healing, trying to be a wife after my body was weaponized against me - it is all so familiar.
Being a survivor of s*xual abuse is complicated. Learning to live after being defiled and gaslit is incredibly challenging. Those of us who have chosen to go public haven’t done so lightly - we, perhaps more than anyone else, know what the truth costs and the price is high.

😮 What if I told you there's a medication that has transformed the lives of veterans and so many others by reducing #PTSD related nightmares?

Let's jump in on this week's #MedMondays💊, and let's vanquish this Nightmare on Elm Street 😱

🧵:
💊 #Prazosin was first developed as an antihypertensive in the 1970s. It wasn't long before it caught the attention of psychiatrists for its remarkable ability to reduce nightmares in those with PTSD. Now, it's a go-to solution for many sleep disturbances! 🌜🛌
🎯 Prazosin shines as a treatment for PTSD-related nightmares, but it doesn't stop there! It's also used to address sleep disturbances in anxiety, depression, and other stress-related disorders, giving patients the rest they need. 🌟🛏️

1- Thread about #autism from an #ActuallyAutistic, that can relatate autism with the #PhantomOfTHeOpera
in 30 seconds.. The thing with #autism is that you, if you are 30 years without diagnosis, that is not the problem, it is to have been living 30 years of a lie =>
2 - So everything is a lie until proven truthful. Feeling abandonment is not a feeing, is a real REAL MANIFESTATION.
We do not feel things, we live things. It is not something like we can stop feeling is for some way or we can chose the result of any manifestation from other. =>
3 - It hurts, because is real, it hurts because you are living a life in which people are getting away because of your #autism. Not because of you. Because if they could been able to understand it, they would know the impact, the result, what they are doing to us. =>

Went to NYC to get castrated. Then was going to head off to Mexico and then maybe Thailand to get more surgery done; maybe even the "final step".

2 weeks before my castration I was on the beach with 2 post-op transgender "women". We were laughing & having a good ole time... til
til they got very serious, looked at me and said, in a sincerity & compassion I'd never heard from them b4, "Don't Do It!, All of my life I thought IF I could just become a woman then I would be happy and I'd find peace & purpose."
he went on to say, "Now I am legally & medically a woman but I am more miserable now than I've ever been. I think of taking my life on a daily basis. Please don't do it to yourself! I just want to die and not exist!" Then the other one looked at me and said...

PTSD is a common phenomenon in ICU survivors, impacting literally millions of people around the world, every year. What do we know about it? A 🧵 1/5
In the late 1990s, research on #PTSD after the #ICU first emerged, with early studies suggesting prevalence rates of 40 to 50 percent, far higher than in combat veterans and other high risk groups. Studies since then have documented rates of 10% to 20% - clearly a problem. 2/5
Risk factors for PTSD in ICU survivors vary but include female sex, young age, a history of delirium, and a history of psychiatric illness. The presence of delusional memories also plays a role. @CIBScenter 3/5

How do we know that our Inner Child is present?
When you are truly feeling your feelings, you are allowing your inner child to be. Your inner child is active when you are being playful, spontaneous, creative, intuitive and surrendering to the spiritual self. When you experience
joy, sadness, anger, fear or affection your child within is coming out.

When the inner child is wounded.
Almost all of us have some inner child healing to do. It is hard for us to be whole and happy adults without bringing the inner child into our lives and thereby healing it.
When we meet our inner child, we often discover that some of our ove, guidance, respect, trust and safety. The lack of these basic needs may have brought about a state of chronic anxiety, fear, shame, anger and despair in our inner child.

Trying desperately not to think about it but having been inadvertently given access to all of my GP notes on system one including psychiatric notes when I was only trying to access a copy of my summary care record is very unsettling. I had deliberately intended never to read 1/
any of my notes. Having worked for the #NHS in some capacity intermittently since 2005 (including time as an admin assistant to a #MHA office) and as a doctor since 2011 I am acutely aware that medical notes can sometimes be brutally dehumanising. I understand it is @rcpsych 2/

FYI: As you talk w/unsheltered people, many are stuck in a recurring #PTSD loop, actively reliving past trauma
They may become- or be perceived as- a threat to others when they are actually struggling to survive/process a sexual assault or other abuse (including police sweeps)
1/
I often hear stories of surviving violence from unsheltered people talking to themselves:
"Get away from me!"
"Leave me alone"
"Get your d-ck out of my face!"
"I don't want to do that..."
It's horrifying- and all too common
+ #SanDiegoPD sweeps can trigger memories of abuse
2/
Imagine someone invades your personal space, gives you orders & refuses to listen to you.
If you've been in this situation before & were injured- those memories can overwhelm your ability to respond to what's happening in "real time"
Result: Behavior based on survival #PTSD

In 2023, let's give #psychotherapy for #mental disorders the status it deserves when examining and regulating novel or "breakthrough" treatments. Our latest comment (@NaudetFlorian @joarhalvorsen Lisa Cosgrove) @TheLancetPsych lays out arguments for this
thelancet.com/journals/lanps…
Trials of new treatments for mental disorders should be comparative (indeed more regulatory trials should thelancet.com/journals/lance…
Patients in the comparison group should receive a 1st line/recommended by guidelines/effective treatment even if *NOT* biological or pharmacological.
The fact that new treatments are touted as breakthroughs after not being compared to the standard of care is a mockery of the patients' time and distress and a negation of ethics and equipoise.
E.g. benchmarking MDMA-assisted psychotherapy for #PTSD

Thoughtful piece

Though yet again the point is missed that services do not only allow restraint & seclusion to be used as a last resort in response to emergencies ("imminent violence") - in some, it's routine

I was never, ever, in any way even remotely violent or even resistant
Once you notice it, it's there in every piece of research & writing on seclusion & restraint - the assumption that these are only ever used as a last resort in response to emergencies

But it's not true

Any & all work that is based on this assumption has failed from the outset
If an analysis/research does not acknowledge that, on some wards, the leadership & culture does not question, facilitates or even expects restraint & seclusion to be used in situations which are NOT emergencies, then it is flawed from the outset

This thread is an attempt to explain the cause, symptoms and treatment of post-traumatic stress disorder. As always, some understandings are subjective and may not represent everybody’s experience. I do my best here, but disagreement is helpful.
Definition: We have to be careful how we define #trauma and thus #ptsd. It is a word that has a specific meaning in medicine, but outside of it can be used for many reasons.
