The mechanisms involved in the anti-inflammatory properties of the #KetogenicDiet remain largely understudied. But we know they are there, and we know they are based at least somewhat on brain immunology. Here is a bit of what we do know. 🧵(1/5)
"glucose metabolism within macrophages and microglia may play a role in KD’s anti-inflammatory effects via regulation of the pro-inflammatory transcriptional activity of the transcription factor Nuclear Factor Kappa B (NFKB). (2/5)
In addition, BHB (a ketone body) is able to mitigate activation-related microglial changes, such as microglial ramification. (3/5)

Dementia is a major public health challenge, affecting around 55 million people worldwide. Unfortunately, it's expected that the number of people living with dementia will increase by 50% over the next decade. (1/10)
Dementia doesn't just appear suddenly one day. It sneaks up on you. There are symptoms of brain fog in the form of mood disturbance and reduced cognitive function years before a diagnosis of dementia is provided. (2/10)
Alzheimer's disease is the most common type of dementia, characterized by memory loss, impaired judgment, and other cognitive difficulties. (3/10)

A lot of you understand that a ketogenic diet is a powerful mitochondrial intervention, but you might be a little unclear on how micronutrient availability can enhance mitochondrial function. 🧵(1/15)
Some of you have or had a bit of increased energy when you started your ketogenic diet, but it wasn't sustained very well. (2/15)
There can be a lot of reasons for that but one common and understandable one, given the shape people are often in when they begin the diet, is micronutrient status. (3/15)

The struggling brain is already trying to shift to ketones for fuel. (1/4)
"The bioenergetic system of the brain is compromised early in the progression of AD and is evident during the prodromal (preclinical) stage of the disease. (2/4)
Glucose hypometabolism and a compensatory shift to an alternative fuel substrate, ketone bodies, have been established as a metabolic phenotype characteristic of the AD brain in both clinical and preclinical studies." (3/4) doi.org/10.1016/j.ebio…

Did your doctor refer to the #ketogenic diet as a fad? Well, I'm sorry. But you're going to have to take a moment to educate them. 🤷‍♀️It's time for a thread. 🧵(1/7)
A common concern is that the ketogenic diet is just a fad weight loss diet & won't help with mental illness. This is far from the truth. (2/7)
The ketogenic diet has been used as a medical intervention for over 100 years to treat seizures in children with epilepsy. It's also used for adults with epilepsy. (3/7)

Talking to your doctor about implementing a #ketogenicdiet to treat your mental illness or neurological disorder? Are they under the impression #ketosis is a medical emergency?
You're going to have to educate them! #neurotwitter #mentalillness #brainhealthmatters
The confusion arises as #Physicians are taught about #Ketoacidosis, a severe condition in people with Type 1 Diabetes or alcoholism. The determining factors for Ketoacidosis are either a lack of insulin with high blood sugar & ketone levels above 5mmol/L or severe malnutrition.
But being in Nutritional #Ketosis is normal & healthy, and should not be equated with Ketoacidosis. It's important to educate ourselves & our #physicians about the difference between these two concepts.

Starting a ketogenic diet? Ease into it. It's more comfortable and practical for most people to go slow, especially if you're on certain meds that affect blood pressure or blood sugar. 🧵(1/5)
Some of you are going to want to go fast and restrict carbs low right away. That's just more of your style. Just know that sustaining this big behavioral change can be easier to maintain when you give yourself a few weeks to get used to what you are doing. (2/5)
Regardless, it's important to set yourself up for success by working with someone who knows you and can help you figure out the best approach for your lifestyle, personality, and likes and dislikes when implementing a #ketogenic diet for #mentalillness. (3/5)

In my clinical experience as a mental health counselor working with clients with bipolar disorder, I have seen a reduction in ##medication usage and stability in #mood 1/8
One of the most exciting parts of this work is seeing my clients imagine themselves as someone who is well. #Bipolar disorder can supersede someone's sense of self, making them believe their symptoms are their personality. 2/8
However, I see the opening of possibilities and improvement in function as they start to dream about doing other things. Some of my clients choose to stay on some medications, but usually, there is a lowering of the dosages and discontinuation of many. 3/8

I don't want you all discouraging someone from asking their oncologist about it when it could be instrumental in saving their life or quality of life. That's not cool. Some of you need to check yourselves. #ethics (3/9)

1/n Warning: long thread approaching (I should have done it as a blog, but that requires too much effort on LSL). Hi @ChrisPalmerMD and @evolutionarypsy. I haven’t forgotten our Twitter convo on the KD in relation to diet and the gut microbiota...
2/n, but I’ve been having too much fun on leave to get to this. Also, I really didn’t even know where to start! The gut microbiota is a complex topic and there’s a lot we do know, but even more than we don’t.
3/n Straight up, to my knowledge there are no credible studies that have directly tested the impact of a #ketogenicdiet (KD) on the #gutmicrobiota (GM) to date.

Colleague and I had this RCT thrown in our face bc suggested a #ketogenicdiet option. Arguement was this RCT shows LCHF ⬆️LDL, therefore bad. Here's a thread about what I think...
(Spoiler, LCHF = good)...
1/ The LDL increase in the LCHF group may not itself be an adverse outcome. There was no subfractionation and it’s likely the increase in LDL and ApoB was driven by large fluffy healthy LDL particles. This assumption is supported an improvement in HDL levels in the LCHF group.
2/ HDL increased on the LCHF group, not the control group. When HDL is high and triglycerides are low, CVD risk is not meaningfully increased. See figure attached from Framingham. Furthermore, HDL (and waist circumference, below) are markers of metabolic syndrome. LDL is not.

I wanted to collect my thoughts on the study released earlier in the week by @KevinH_PhD. It turned into this blog-sized thread.

The study compared a high-carb, plant-based diet to a low-carb, omnivorous diet in a 4 wk. cross-over study.

The results created quite a stir.

1/
Participants were randomly assigned to a plant-based diet or low-carb diet for two weeks. They then switched to the other diet for the following two weeks.

Both groups lost weight but participants on the plant-based diet ate less and lost more fat, despite higher insulin.

2/
This triggered some to question whether the carb-insulin model was dead. CICO enthusiasts took a few victory laps, bashing low-carbers as foolishly fixated on carbs and beyond reason.
